Thinking Spine Surgery for Disc Herniation? Think Again . . .

A recent study on lower back surgery demonstrated discouraging results regarding returning to gainful employment. The study showed that of those patients who chose spine surgery over non-surgical intervention for damaged spinal discs, only 26% of the spine surgery patients returned to work, compared to 67% of the non-surgical group. The article was posted at www.msnbc.com and can be referenced by clicking here.  The article goes on to demonstrate the increase in the use of strong pain medications after spine surgery, to go along with the increased disability reate.  Although spine surgery can be a life saver for some, statistical studies continue to prove that only 1-2 out of 10 spine surgery patients actually are much better off after the surgery than before.  This is mostly due to the complexity of the spine/ disc / joint / musculoligamentous / neurological network of tissues, alteration of biomechanical functioning of the spinal units, and the collateral damage that occurs to important neuromusculoskeletal components during and after these highly invasive surgeries.

Most patients who understand the invasiveness of surgical intervention realize that spine surgeries typically either remove important functional supportive tissue, or alter the anatomy and physiology altogether by fusing vertebral segments that were designed to be in motion. Surgical intervention as a treatment for disc herniation often results in removal of viable tissues, resulting in weakening of the supportive integrity of the spine and discs, setting the entire biomechanical system on a course for failure.

If you think of a spinal disc like a car tire, the thick outer rubber that contacts the road can wear down, develop cracks and tears which either result in a slow leak or sudden blow out of the tire. Similarly, the inner shock absorbing gel of our spinal discs can either gradually lead our or damaged disc, or the disc can "blow out" or herniate all at once.  Rather than repairing the lead or the blow out (herniation) must surgical techniques either remove portions of the disc (or tire as in our analogy) making the disc (or tire) less durable and more likely to fail further. An auto mechanic wouldn't advise "tire surgery" in most cases, but would usually recommend new tires. The problem with this analogy is that we can't just order a "new set of discs" for our spines . . . or can we?

With advances in non-surgical technologies and regenerative medicine techniques, spinal disc regeneration which was once considered impossible, is now being recognized as a viable solution in many cases. Stem cell therapies, nutraceuticals, and laser medicine are just a few of the techniques being studied and utilized. Osteoporosis, Hip Fractures, and What You Can Do To Avoid Them!

The Sobering Facts About Hip Fracture

Many people fear hip fractures, and with good reason. Of all types of fractures, hip fractures have the most significant impact on the quality of life.

Alarmingly, close to 25 percent of those over age 50 who break a hip will die within a year. In addition, 40 percent are unable to walk independently a year after they've broken a hip, and 60 percent are unable to perform basic activities of daily living, such as dressing themselves. Because of these devastating consequences, preserving and boosting bone mass and preventing falls and fractures are crucial.

A hip fracture usually occurs when a person falls from a standing position, with the hip taking the impact of the fall, although less traumatic falls also can cause hip fractures. More than 90 percent of the 350,000 hip fractures that occur each year in the United States are the result of a fall. The rate of hip fracture begins to increase at age 50, doubling every five to six years. Nearly half of women who reach age 90 have had a hip fracture. Men account for nearly 30 percent of hip fractures.

The risk factors for a hip fracture are:

    • being older than age 65
    • being female
    • having a family history of fractures later in life
    • low body weight
    • poor nutrition
    • smoking or excessive alcohol use
    • mental impairment
    • poor eyesight
    • unsteady balance
    • frailty

What to expect. The symptoms of hip fracture are not subtle. They include pain in the groin or buttock, inability to put weight on the leg on the affected side, or pain when trying to bear weight. Hip fractures may also interrupt the nerves or blood vessels that supply the leg. All hip fractures require surgical repair or replacement, and treatment is often required for injuries related to the fall, such as head injuries.

Additionally, it is vitally important to note that the reported overall mortality (or death) rate for hip fractures is 15-20% (1 out of 5). Yet in older persons this can increase to 36% (over 1 out of three) over the year following hip fracture. These numbers are significant, and should motivate anyone at risk or potentially at risk of hip fracture due to osteoporosis or osteopenia to seek out appropriate medical attention. However, one must also be fully aware of the risks associated with osteoporosis medications like; Fosamas, Boniva, or Actonel.

Concussion, More Common Than Thought

With school back in session and many children and teens getting back into school sports programs, I thought it would be a good idea to address a subject that has been gaining a lot of media attention lately.

That subject is: Concussion, and it's defined as: an immediate and transient loss of neuronal function secondary to trauma. It's important to realize that concussion often goes unrecognized in adolescent sporting activities for 2 primary reasons. First, athletes who suffer concussion have admitted in recent studies that they do not report their concussive events due to their concern over letting the team down or being taken out of play. Second, concussions are often not reported due to a lack of understanding of what constitutes a concussion. Both of these reasons are cause for concern and be should be addressed with greater clarity between coaches, athletes and their parents. It is a serious matter and could make a difference between life and death or permanent neurological impairment.

Concussion is not confined to just "getting knocked out" or losing consciousness, although that is the most prominent feature of a severe head trauma. Concussions are actually more frequently sustained (85% of the time) without a loss of consciousness. So here is a simpler definition of what is meant by a "loss of neuronal function secondary to trauma". 

I will list the 5 most prominent symptoms: (one or more usually indicate concussion)

1. Loss of Consciousness (not required to be classified as a concussion)
2. Loss of Memory, (of events before or after the trauma)
3. Confusion with possible slurring of speech.
4. Decreased motor skills, (uncoordinated movements, loss of balance, etc.)
5. Emotional or personality changes. (acting noticeably "different" than usual)

Concussion can occur without actual blunt trauma to the head. If an athlete sustains a full body trauma in the form of a collision with another athlete or object, or from a fall to the ground, without actually striking the head, there can still be significant trauma to the brain that results in a concussion, or even bleeding into or around the brain.

The key factors involve immediate recognition of a concussive event through proper observation and attention following a trauma. Properly trained, the coach, trainer, or fellow team mates can contribute significantly to recognizing the trauma an athlete endures, and then making sure they receive the proper attention through medical evaluation with a team physician or specialist right away. This sequence is of utmost importance to the injured athlete, as concussive injuries can be lethal, especially when they occur more than once!  Second Impact Syndrome is where a second concussion can lead to bleading within or around the brain, resulting in permanent paralysis, other severe loss of neurologic function, or death.

A well trained team doctor can immediately administer a Standardized Assessment of Concussion (SAC) that only takes 5 minutes. The testing includes grading the concussion in terms of orientation, memory, neurologic function, Loss of Consciousness duration, amnesia (degree of retrograde or anterograd), sensation, coordination, concentration, exertion maneuvers,  and delayed recall, to name a few. This would be followed by appropriate neurological evaluation and testing, resting physically and psychologically (cognitive function, ie. schoolwork, video games, and internet), and adherence to carefully constructed "return to play" guidelines. As mentioned above, these types of injuries must be taken very seriously due to their potential for becoming lethal.

Reversing Osteoporosis Safely and Effectively at the South Coast Spine Center of San Diego

Osteoporosis is often referred to as "the silent epidemic", affecting over 10 million Americans. It is a potentially serious disease that can result in debilitating and sometimes life threatening stress fractures.  Supplementing the diet with Calcium, Magnesium and Vitamin D3, while increasing weight bearing exercises are helpful, but oftentimes are just not enough to reverse the process of bone weakening.

Risk factors you can change

• Low calcium intake. A lifelong lack of calcium plays a major role in the development of osteoporosis. Low calcium intake contributes to diminished bone density, early bone loss and an increased risk of fractures.
• Tobacco use. The exact role tobacco plays in osteoporosis isn't clearly understood, but researchers do know that tobacco use contributes to weak bones.
• Eating disorders. Women and men with anorexia nervosa or bulimia are at higher risk of lower bone density.
• Sedentary lifestyle. People who spend a lot of time sitting have a higher risk of osteoporosis than their more-active counterparts. Any weight-bearing exercise is beneficial for your bones, but walking, running, jumping, dancing and weightlifting seem particularly helpful for creating healthy bones.
• Excessive alcohol consumption. Regular consumption of more than two alcoholic drinks a day increases your risk of osteoporosis, possibly because alcohol can interfere with the body's ability to absorb calcium.
• Corticosteroid medications. Long-term use of corticosteroid medications, such as prednisone, cortisone, prednisolone and dexamethasone, is damaging to bone. These medications are common treatments for chronic conditions, such as asthma, rheumatoid arthritis and lupus, and you may not be able to stop taking them to lessen your risk of osteoporosis. If you need to take a steroid medication for long periods, your doctor should monitor your bone density and recommend other drugs to help prevent bone loss.
• Other medications. Long-term use of aromatase inhibitors to treat breast cancer, the antidepressant medications called selective serotonin reuptake inhibitors (SSRIs), the cancer treatment drug methotrexate, some anti-seizure medications, the acid-blocking drugs called proton pump inhibitors and aluminum-containing antacids are all associated with an increased risk of osteoporosis.

Spinal Disc Regeneration; How It Is Being Done Today!

Pluripotent Stem Cells (recently called "Biology's New Supermodel) have have been my favorite "Biological Subject" since the days of my pre-med biology classes. The most exciting thing to me is how we continue to learn more and more about their therapeutic usage as researchers and clinicians are implementing regenerative medicine procedures in their efforts to "heal and relieve" the causes of disease.

Outdated beliefs are now being challenged and overturned with the advent of "regenerative medicine", and the utilization of our own stem cells, ASC's (Adult Stem Cells) to stimulate regeneration and repair of damaged tissue. Here is a brief overview of one recent study that documents what some of my colleagues have known for years, that spinal discs can be regenerated through the activity of stem cells.

Study Design. Marrow mesenchymal cells (MSCs) contain stem cells and possess the ability to regenerate bone, cartilage, and fibrous tissues. Here, we applied this regenerative ability to intervertebral disc regeneration therapy in an attempt to develop a new spinal surgery technique.
Objective. We analyzed the regenerative restoration ability of autologous MSCs in the markedly degenerated intervertebral discs.
Summary of Background Data. Fusion for lumbar intervertebral disc instability improves lumbago. However, fused intervertebral discs lack the natural and physiologic functions of intervertebral discs. If intervertebral discs can be regenerated and repaired, then damage to adjacent intervertebral discs can be avoided. We verified the regenerative ability of MSCs by animal studies, and for the first time, performed therapeutic intervertebral disc regeneration therapy in patients and obtained favorable findings.
Methods. Subjects were 2 women aged 70 and 67 years; both patients had lumbago, leg pain, and numbness. Myelography and magnetic resonance imaging showed lumbar spinal canal stenosis, and radiograph confirmed the vacuum phenomenon with instability. From the ilium of each patient, marrow fluid was collected, and MSCs were cultured using the medium containing autogenous serum. In surgery, fenestration was performed on the stenosed spinal canal and then pieces of collagen sponge containing autologous MSCs were grafted percutaneously to degenerated intervertebral discs.
Results. At 2 years after surgery, radiograph and computed tomography showed improvements in the vacuum phenomenon in both patients. On T2-weighted magnetic resonance imaging, signal intensity of intervertebral discs with cell grafts was high, thus indicating high moisture contents. . . lumbar disc instability improved. Symptoms wer alleviated in both patients.
Conclusion. The intervertebral disc regeneration therapy using MSC brought about favorable results in these 2 cases. It seems to be a promising minimally invasive treatment. (End of Abstract)

The Power of Positive Psychology: How Focusing on What’s Right Can Help Fix What’s Wrong

This Morning's Johns Hopkins Medicine Health Alert had some great information on Mental Health and how our thoughts affect our feelings. So I've reposted a brief outline of the article below.

Most forms of psychotherapy are based on discussing and analyzing negative thoughts and feelings. But some experts say this approach overlooks a larger issue: how to use psychology to emphasize the positive and increase happiness, not just fix problems.

While alleviating the symptoms of mental disorders is certainly essential, proponents of positive psychology argue that the search for mental health shouldn't end there. As Martin Seligman, Ph.D., one of the leading researchers in positive psychology explains, current treatment aims to bring people from –5 to 0 (on a scale of –5 to +5); ideally, psychology should help us get from 0 to +5. In other words, the absence of mental illness is not the same thing as good mental health.

How Positive Psychology Works. Positive psychology involves more than just thinking happy thoughts. Positive psychology focuses on the strengths and virtues that help people thrive, such as courage, gratitude, compassion, resilience and creativity. While most therapists attempt to "fix what's wrong," positive psychologists also want to "build what's strong."

Therapists who practice positive psychology help individuals see their strengths and virtues and find ways to foster them. They also help people identify positive experiences and the circumstances that brought them about, as well as encourage behaviors that give life a sense of meaning and purpose.

Here are some positive psychology techniques you can try:

• Identify and use signature strengths. Write down your top five strengths and try to use them more and in new ways each day.
• Keep a gratitude journal. Often recommended by Oprah Winfrey, this positive psychology technique involves writing down each evening three good things that happened to you and noting why you think they happened. Many people focus on negative emotions or events and ignore the positive ones. Keeping a gratitude journal is a way to shift your focus to the more positive aspects of your life and to reflect on them.
• Express appreciation to other individuals. Extend your gratitude to the important people in your life.
• Perform acts of kindness. Turning your focus to improving the lives of people around you can increase your own happiness. Try to help others in ways both random (like holding the door open for someone or letting someone go in front of you in line) and planned (such as volunteering or donating blood). Healing at the Speed of Light!

Lasers have been used in medicine since the early 1960's. Medical lasers were more widely studied and utilized in Europe and Russia in the early years, but have quickly been gaining more acceptance and utilization here in the US over the last 10 years as Americans seek out safer alternatives to drugs and surgery. Perhaps the most amazing discovery with medical lasers have been the "regenerative" or healing effects they produce. Given the proper wavelengths, power, and protocol, healing lasers are bringing not only pain relief, but regenerative healing to more people than ever before. The following article on Low Level Light Therapy is from the North American Association for Laser Therapy (NAALT) and provides a good introduction to the science of Laser Therapy.

Low Level Light Therapy (LLLT)
Low Level Light Therapy - Biological reaction to light is nothing new. We normally experience this through our eyes which are obviously photosensitive – our vision is based upon light hitting our retinas and creating a chemical reaction that allows us to see. We experience this with vitamin D synthesis in our skin – due to sunlight creating biochemical reactions. We experience this with suntans – due to the light of the sun stimulating our melanocytes to proliferate and give us “brown” skin. Light reacts chemically with other things as well. Hydrogen peroxide is stored in dark bottles so that light can’t get in and initiate a chemical degradation. Light can photo-chemically affect all things – eventually – one way or another.

Low level laser therapy does not induce heating in tissue like surgical or aesthetic lasers. It’s all in the photochemical reactions of certain wavelengths of light with our cell membranes, cellular organelles and enzymes. Compromised cells respond better than healthy cells to photochemical reactions so that brief treatments with low level light on damaged tissue can induce a complex chain of physiological reactions to enhance wound healing and tissue regeneration, reduce acute inflammation, treat chronic pain and even to make our skin look younger and more vibrant.

Red light accelerates hair growth. It has also been shown to biochemically melt fat cells. Low level infrared light has been shown to significantly enhance the recuperation of stroke victims by shining a low level laser that you can’t even see or feel on top of your head for a few minutes at a time. Sounds like magic doesn’t it? – well, it is. It’s light and it does seem to have magical properties. The science of this “magic” has become increasingly better explained in recent years and it is apparent that this is a valid therapy for many conditions. In spite of the scientific validity of LLLT there are still many commercial claims for devices that are not well supported and some are in fact just plain incredulous. The message to take from this though is to not throw out the baby with the bathwater when confronted with some of the fringe claims of LLLT. It has substantial merit as a valid therapy.

It has been called many names over the years including biostimulation, LLLT, cold laser therapy, and more recently photobiomodulation. Low level lasers (mostly diode lasers in the 10-200 milliwatt range) have been the primary instruments but there are also non-laser light emitting diodes (LED’s) and other light sources that are used.

In the U.S. one sees these low level red, blue, yellow and infrared lights used in sports medicine clinics as part of the physical therapy to enhance healing of joint and soft tissue injuries such as arthritis treatment, carpal tunnel syndrome or athletic sprains and injuries. It has been used prior to radiation therapy in breast cancer to prevent the inflammatory dermatitis that is frequently associated with this. In MediSpas these devices are used for periodic treatments to enhance skin rejuvenation.

The devices are employed as one of the therapy options in pain clinics to treat chronic pain, including shingles pain. Some units are even being sold to the home market for general treatment of muscular and joint “aches and pains”. It’s used for things like low back or cervical neck pain, and is good to consider for use in hard to manage problems like fibromyalgia.

Its major use however has been outside of the United States, and outside of FDA restrictions, where it has been a long standing therapy for wound healing – especially chronic wounds such as decubitus or diabetic ulcers that are otherwise hard to heal. In research, LLLT appears to enhance nerve regeneration in spinal cord injuries.

There is a related type of low level laser use that has been widely publicized, but does not appear to work by the same “photobiomodulation” mechanisms. That’s the “stop smoking” and “weight loss” laser clinics that appear all over the United States. Aside from some strong testimonial support from treated clients, there does not currently appear to be much scientific evidence establishing the mechanisms of action of this type of laser therapy. That doesn’t mean that it won’t eventually be found to be valid, but the evidence is just not there yet and it should not be confused with conventional LLLT. In “stop smoking” or “weight loss” the light is directed at certain anatomical acupuncture points that correspond to nerve networks. Regular LLLT for tissue regeneration or chronic pain treatment does not require this. If the laser therapy proves to have an actual effect to help people stop smoking then it is probably an acupuncture type of therapy that simply uses low level lasers instead of needles to induce these changes, and it would then beg the question of whether the needles wouldn’t then be more effective to begin with (although many people would prefer the light over penetrating needles).

The potential applications of low level laser therapy are very widespread, and too broad to list in much detail in this introductory article. You should now have a better idea of what LLLT is however, and differentiate it from related low power laser uses like stop smoking. When you are ready to research it more you’ll find prolific material that has been generated over the years with Russia having probably the most experience, but also Europe, South America and the Middle East.

Peripheral Neuropathy Relieved with DRX9000 Spinal Decompression

We consider ourselves to be so fortunate here at the South Coast Spine Center, to consistently witness the healing and life restoration process in our patients. From time to time we like to share the life changing stories of our growing list of "DRX Success" testimonials. Here's one that addresses improvement of a patient who was previously diagnosed with peripheral neuropathy (pain, weakness, and loss of sensations due to nerve damage) that doctors originally thought was caused by diabetic issues, but who experienced significant relief with our DRX9000 Spinal Decompression therapy procedure for his lumbar spine.  Prepare to be inspired with hope . . . 

I was very skeptical when a close friend first recommended that I should see Dr. Bell. Every time I ever mentioned my back problems to her, she strongly told me that I needed to go see him.  I probably never would have, but my back got so bad, I had to do something.  It finally got so bad (7-10 level pain out of 10 being unbearable pain) that I couldn’t even get dressed to go to church (couldn’t bend, twist, or pick anything up).  Then a couple of other good friends that were patients of Dr. Bell’s also told me of their relief of their back problems. 

When we made the decision to go ahead with the series of treatments, I decided to really commit to it and see it through.  Starting with the first treatments, I could feel the difference with my back.  Gradually I had less pain and felt more and more flexibility and relief from the constant back pain that I had.  The treatments were totally painless and I always felt very relaxed during and after each treatment.

After the treatments I was curious about how long it would last.  It has now been over four months since the treatments were completed and I am still pain free and have much improved flexibility. Another side benefit is my peripheral neuropathy that I had in my legs is much improved.  I had been diagnosed as having peripheral neuropathy for years, because I’m a “borderline diabetic”, whatever that is. All the time it was caused by compressed nerves in my lower back.

Headaches . . . What To Do AND Who To See About Them!

            Before we address the subject of headaches, here are some important facts about the largest drugless healthcare profession in the world:

            Did you know that in 1976, Dr. Chester Wilk and four other Chiropractors filed a lawsuit against the American Medical Association (AMA)?

             The suit claimed the American Medical Association (AMA) had participated for years in an illegal conspiracy to destroy Chiropractic. 

             On August 24, 1987, after endless wrangling in the courts, U.S. District Court judge Susan Getzendanner ruled the AMA had engaged in a “lengthy, systematic, successful and unlawful boycott” designed to restrict cooperation between MDs and Chiropractors in order to eliminate the profession of Chiropractic as a competitor in the United States health care system.

             And here’s something interesting:  During the case, the AMA took the stance of protecting patients. Chiropractors, however, showed the court Workmen’s Compensation Bureau studies validating Chiropractic care. 

             More specifically, studies comparing Chiropractic care to care by  medical physicians were presented showing Chiropractors were “twice as effective as medical physicians, for comparable injuries, in returning injured workers to work at every level of injury severity.”

             The settlement of the suit included an injunctive order in which the AMA was instructed to cease its efforts to restrict the professional association of chiropractors and AMA members. The AMA was also ordered to notify its 275,000 members of the court's injunction. In addition, the American Hospital Association (AHA) sent out 440,000 separate notices to inform hospitals across the United States that the AHA has no objection to allowing chiropractic care in hospitals.

Now to address the subject of headaches  and what to do about them . . .

                     If you suffer from headaches, you are not alone.      

The number of people who suffer from headaches every year is staggering. 

A large percentage of these headache are “tension-type” headaches. Stress of daily living is a major source of neck and upper back tension that can cause tension-type headaches and even migraines.

        Migraine headaches are perhaps the most "well known" of all headaches, and are often the least understood. These debilitating headaches are usually caused by circulatory problems to the brain. If the circulation of blood is restricted through prolonged constriction of the blood vessels to the brain, they will eventually fatigue and "over-dialate" resulting in increased "intracranial pressure" or pressure within the head from increased blood flow.  Migraines can also be associated with restricted blood flow to the brain due to problems in the neck.

        Another common type of headache is called “cervicogenic" which means that it originates in the neck, usually from neck tension that can be caused by stiff joints, spinal misalignment, or altered joint biomechanics caused by arthritis or disc injuries.  Headaches can also be a blend of both "tension" and "cervicogenic". A major study on headaches published in Germany linked approximately 80% of all headaches to tension in the muscles of the neck.

        This is important because a 2001 Duke University study showed Chiropractic care helped both these types of headaches  both in headache frequency and severity.  The study also showed Chiropractic care worked better than many other treatments.

Chiropractic Treatment of Chronic ‘Whiplash’ Injuries

Medical research has consistently proven that Chiropractic Treatment for Chronic Whiplash injuries is superior to traditional non-chiropractic care. ie. medication, rest, physical therapy. In the study summarized below from the Medical Journal, "Injury", 93% of the chronic whiplash patients reported improvement in their symptoms with Chiropractic care! Scientific Chiropractic Treatment of Chronic ‘Whiplash’ Injuries

Injury, Volume 27, Issue 9, November 1996, Pages 643-645

Forty-three percent of patients will suffer long-term symptoms following ‘whiplash’ injury, for which no conventional treatment has proven to be effective.  

A retrospective study was undertaken to determine the effects of chiropractic in a group of 28 patients who had been referred with chronic ‘whiplash’ syndrome.

The severity of patients' symptoms was assessed before and after treatment using the Gargan and Bannister (1990) classification.

 Twenty-six (93%) patients improved following chiropractic treatment. 

The 28 patients in this study had initially been treated with anti-inflammatory medication, soft cervical collars and physiotherapy. These patients had all become chronic, and were referred for chiropractic at an average of 15.5 months (range was 3 – 44 months) after their initial injury. 27/28 (96%) patients were classified as category C (Intrusive) or D (Disabling) symptoms at the time prior to initial chiropractic treatment.

Following chiropractic treatment, 93% of the patients had improved: 16/28 (57%) by one symptom group and 10/28 (36%) by two symptom groups. 

KEY POINTS FROM THIS ARTICLE

 1) 43% of those injured in whiplash will experience long-term symptoms. In this study, at least one patient had ongoing symptoms 3 years 8 months following whiplash injury.

2) “If [whiplash] patients are still symptomatic after 3 months then there is almost a 90% chance that they will remain so.”

3) “No 'conventional' treatment (ie.  pain meds, physical therapy, exercise, rest, immobilzation, etc.) has proven to be effective in these established chronic cases.”

4) “Spinal manipulation is a high-velocity low-amplitude thrust to a specific vertebral segment aimed at increasing the range of movement in the individual facet joint, breaking down adhesions and stimulating production of synovial fluid.”

5) In this study, chiropractic improved the symptom category of 93% of chronic whiplash patients. This is particularly important considering that 96% of these 28 patients had intrusive symptoms that handicapped their work and leisure activities and required frequent use of pain drugs, or they were severely disabled, having lost jobs and required continued medical interventions with constant use of pain drugs.

6) “The whiplash syndrome is a cause of long-term symptoms for which conventional medicine has failed to discover an effective treatment.”

7) Chiropractic has been shown to be advantageous compared to conventional medicine in the treatment of low back pain.

8) Complications from cervical manipulations are rare, and when they are reported in the literature, they often “arose as a result of spinal manipulation performed by non-chiropractors, who had been misrepresented in the literature as being trained chiropractors.”

Whiplash Facts; Why Your Neck Hurts After A Car Accident

The most common causes of “whiplash” are injuries that arise from automobile accidents or motor vehicle collisions (MVC’s). So, let’s chat about why and how this happens in a “typical” MVC.  You are stopped at a red light, patiently waiting for the light to turn green and suddenly, you hear the screech of tires followed by a sudden jolt as the car from behind collides into the back of your vehicle.  By reflex, you may turn your head to the right to look in the rearview mirror to see what is happening.  Even if you see the inevitable collision prior to the impact, the sudden jolt occurs so fast that you really don’t have a chance to adequately brace and you feel yourself being forced back into the seat and headrest followed by a rebound forwards. 

Since you always wear your seatbelt, you feel the restraint across your chest and lap belts tighten as you’re propelled forward.  The seat belt saves your life by stopping you from hitting the steering wheel or from being launched forward through the windshield. However, at this point of the impact, the soft tissue damage (ie. torn muscles, ligaments and tendons, compressed or ruptured nerves and discs, and bruised internal organs) has been done! 

This ALL occurs in less than 500 milliseconds.  You cannot voluntarily contract your muscles this fast, which means even if you had time to prepare yourself for the impact by bracing, you can’t stop the whiplash effect! (See diagram below for time sequence)

 In a recent study, it was found the muscles in the front of the neck contract first at about 100 ms, which is 25 ms too late to prevent ligament or muscle damage, and they reach their peak stretch at 150ms (see 3rd from the left picture above).  The muscles in the back of the neck start contracting soon thereafter but are injured more than the muscles in the front of the neck around the 300ms point.  The reason for this is because as the head rebounds forwards, the muscles in the back of the neck are in the process of tightening up or shortening at the same time they are being stretched – (your body's attempt to protect itself under normal forces) but NOT a good combination under the excessive G-Forces experienced during a car crash!

This is one reason why many people injured in MVC’s often complain of greater intensity of neck pain in the back of the neck than in the front of the neck. Patients often present to our offices following a car accident with neck pain in the front, sides, and back of the neck, and in more severe cases the pain will radiate into the shoulders, arms and even to the hands and fingers, indicating injury to the deeper spinal nerves. Some patients will also have pain when swallowing due to the stretching trauma to the esophagus during the collision. 

This also helps explain why headaches are common symptoms associated with whiplash as the upper 3 nerves that exit the top of the spine in the neck go into the head/scalp and are compressed or squeezed by the tight muscles in the back of the neck when they are injured which results in headaches. The spinal discs can also be injured by Whiplash, requiring proper diagnostic studies that may include MRI, Dynamic Motion EMG, and other more specific studies.

Whiplash, and Whiplash Associated Disorders (WAD) can be quite serious and debilitating. They should be taken seriously, and anyone suffering from the injuries of a car accident should be seen by a physician who specializes in these types of injuries. Too often, a primary care doctor will simply prescribe pain medication and some rest, ignoring the potential severity of their patient's symptoms following a car crash, neglecting important treatment that could save the patient from future pain and disability.

Back and Neck Pain? Here's One Very Successful Solution!

Back pain and neck pain are two of the most common maladies suffered by humanity. More time and enjoyment in life are lost due to these conditions than perhaps from any other type of ache or pain, according to US Department of Health statistics.

Sleep "Hygiene Therapy" . . . Can It Help Heal Your Back Pain?

Most of our patients seeking relief of chronic and severe back or neck pain at the South Coast Spine Center are also suffering from sleep deprivation due to their pain. This usually results in a vicious sleep loss cycle, creating more pain, and even less sleep.  Thankfully, our patients start sleeping better very soon after starting care due to the pain relief that they experience with our advanced treatment technologies. Since proper sleep is vital to healing and pain relief, here is a great resource for improving your sleep!

Good “hygiene” is anything that helps you to have a healthy life. The idea behind sleep hygiene is the same as dental hygiene. Dental hygiene helps you stay healthy by keeping your teeth and gums clean and strong. Sleep hygiene helps you stay healthy by keeping your mind and body rested and strong. Following these tips will help you sleep better and feel your best.

Dental hygiene can even be a part of your sleep hygiene. It is easier for you to fall asleep at night if you have bedtime “rituals.” These are things that you do every night just before going to bed. Brushing and flossing your teeth is a good example of this kind of ritual. Both your dentist and your doctor will approve if you do this every night.

Here are some tips for how you can improve your sleep hygiene:

1. Don’t go to bed unless you are sleepy.
   If you are not sleepy at bedtime, then do something else. Read a book, listen to soft music or browse through a magazine. Find something relaxing, but not stimulating, to take your mind off of worries about sleep. This will relax your body and distract your mind.
2. If you are not asleep after 20 minutes, then get out of the bed.
   Find something else to do that will make you feel relaxed. If you can, do this in another room. Your bedroom should be where you go to sleep. It is not a place to go when you are bored. Once you feel sleepy again, go back to bed.
3. Begin rituals that help you relax each night before bed.
   This can include such things as a warm bath, light snack or a few minutes of reading.
4. Get up at the same time every morning.
   Do this even on weekends and holidays.
5. Get a full night’s sleep on a regular basis.
   Get enough sleep so that you feel well-rested nearly every day.
6. Avoid taking naps if you can.
   If you must take a nap, try to keep it short (less than one hour). Never take a nap after 3 p.m.
7. Keep a regular schedule.
   Regular times for meals, medications, chores, and other activities help keep the inner body clock running smoothly.
8. Don’t read, write, eat, watch TV, talk on the phone, or play cards in bed.
   
9. Do not have any caffeine after lunch.
   
10. Do not have a beer, a glass of wine, or any other alcohol within six hours of your bedtime.
    
11. Do not have a cigarette or any other source of nicotine before bedtime.
     
12. Do not go to bed hungry, but don’t eat a big meal near bedtime either.
    
13. Avoid any tough exercise within six hours of your bedtime.
    You should exercise on a regular basis, but do it earlier in the day. (Talk to your doctor before you begin an exercise program.)
14. Avoid sleeping pills, or use them cautiously.
    Most doctors do not prescribe sleeping pills for periods of more than three weeks. Do not drink alcohol while taking sleeping pills.
15. Try to get rid of or deal with things that make you worry.
    If you are unable to do this, then find a time during the day to get all of your worries out of your system. Your bed is a place to rest, not a place to worry.
16. Make your bedroom quiet, dark, and a little bit cool.
    An easy way to remember this: it should remind you of a cave. While this may not sound romantic, it seems to work for bats. Bats are champion sleepers. They get about 16 hours of sleep each day. Maybe it’s because they sleep in dark, cool caves.

A good night's sleep is absolutely essential to optimal health, and can make a huge difference in your body's ability to heal completelty. Combined with "the right" treatment for that chronic and severe back pain, neck pain, or sciatica leg pain, painfully sleepless night will become a faint memory of the past. Headache Sufferers . . . Suffer No More!

Often times our patients at the South Coast Spine Center graciously want to share their successful outcomes with our customized, "high-tech" treatment programs, with the hope that their success will encourage others to find healing and relief with our methods as well. "The Days of My Chronic Headaches Are Gone!"

BEFORE DRX:  When I first consulted with Dr. Bell, I was plagued by daily headaches, along with a tingling sensation in my jaw, shoulder and down my right arm. I was unable to sleep or sit comfortably, as I was always trying to change my position to alleviate these symptoms. I have tried chiropractic, physical therapy and had been taking pain medication daily.  I was told that surgery was the only option left for me.

As you can see, I’ve put a whole bunch of pictures of some of the world’s greatest athletes, actors and celebrities on the top of this letter. Why have I done this?

I have done this because they all have something very important in common with YOU.

Chiropractors Monterey – ‘Tis the season for colds and sinus infections. I just got over one myself. I rarely get sick, but it happens to the best of us, especially when you have little kids who are regular germ factories! Anyways, it is nice to see that more and more people are becoming more educated and aware that taking antibiotics on a regular basis can actually weaken your immune defense and make you increasingly resistant to antibiotics when you really need it.

I’m sure there are lots of “at home remedies”, but here is one tip to help…

- 1 Cup of luke warm water + 1/4 teaspoon of salt
- Syringe or bulb is needed
- Tilt head back
- Close throat
- Squirt into one nostril while holding other nostril closed
- Shake head
- Blow
- Repeat 2 times on each side.

My Large Disc Herniation Is Gone!

We love to hear and read about the success stories that our patients at the South Coast Spine Center share with us. They serve to inspire us and future patients who might have given up hope for recovery. This patient, Rocio Mora, presented with the one of the largest lumbar disc herniations I've seen. She responded very well to our care and experienced significant relief, recovery, and life restoration. You can read her story below.

I had 2 years of constant severe lower back pain that went into my right leg to my foot, with numbness. Sometimes I couldn’t even tie my own shoes. I had 2 cortisone shots but they didn’t last for even a month. My neurologist told me that I needed back surgery/fusion after physical therapy didn’t work. My pain got worse. I had 3 MRI’s of my low back and the last one showed a very large herniation in one of my discs. Another neurologist told me to try the DRX9000 treatment at Dr. Bell’s office before having surgery.

I was worried that my pain would get worse, but the DRX treatment made sense. I didn’t notice any change during the 1st 10 treatments. On the 15th visit, I didn’t need any more pain medication. Toward the end of my treatment, my leg pain went away completely and my back pain was almost entirely gone. I had to drive a long distance to get to Dr. Bell’s office, 2 – 3 hours each way, and I think that slowed my recovery a bit. I felt relief with the DRX treatments, but even more relief came with the extra physiotherapy provided by Dr. Bell’s staff.

Now I feel much better and stronger, especially in my legs. I feel relieved and more secure and confident knowing that my leg will not give out on me any more.I got another MRI after completing my DRX treatments and the large disc herniation is GONE!  I herniated a disc in my low back 17 years ago which caused a 75% decrease in  feeling in my right foot and calf.  Then, I re-injured my back when picking up a water bottle.  I felt a sharp pain that dropped me to my knees.  I saw Dr. Bell’s ad and decided to check out his treatment.  The MRI that Dr. Bell ordered for me showed more damage in my spine that I thought I had.  My neurosurgeon had told me that I had the spine of a 60 year-old, and I was only 30.  My 8 hour work day took me 13 hours to complete.

Burnout Busting "Decompression" Tips . . . From A Decompression Specialist!

Do you ever feel like you just don't have enough time to "get it all done" every day? Do you find yourself putting off too many of the little things, and sometimes the important ones too? Are you on the road to burnout, or have you already arrived?

If you're like most people, you probably wish you had more hours in the day to take care of everything on your "to do" list. In order to have optimal health and happiness, we need margin. Without it, we feel "compressed, stressed, pressured" and just plain unhealthy. Excessive or prolonged stress in any part of our lives and/or relationships leads to compression, dis-ease and sickness.

So, here are some important tips on how to "lighten your load", decompress, and create more margin in your life!  Take one or two from this list today and see if it makes you feel a little "less stress".

MARGIN CREATING – BURNOUT BUSTING TIPS

1. Schedule regular social activities
Remember when you used to spend time with people you were neither working with nor sleeping with? You watched movies, ate meals, played games, and went on trips. You were active and you had fun!
You can regain some of that emotional fulfillment by contacting some of your old pals and scheduling regular activities.
2. Follow a fitness plan
If you want to avoid burnout, resurrect that New Year’s Resolution and figure out what it takes to get you exercising on a regular basis. Apart from all the physical benefits of exercise, you’ll enjoy the mental satisfaction of knowing that you’re taking good care of yourself again.
3. Pursue a hobby
Pick a hobby that has little or nothing to do with what you spend most of your week doing and pursue it with passion! A hobby that uses an entirely different skill set can provide your heart and mind with a satisfying break from the weekly grind and set you on a good path for increased productivity.
4. Volunteer
Nothing brightens the soul or warms the senses like giving to another for no reason other than to give. If you’re feeling run down by life, seek out somebody less fortunate than yourself and work to help them.
5. Write a manifesto
Have you forgotten what you want out of life? It’s easy to lose track of time and even easier to forget about what makes us glad to be alive. What can you do to bring back that focus? Take a day or perhaps an entire weekend and write a manifesto, a declaration of purpose, for yourself.
6. Ask for help
Whether your struggle is with a particular part of a project or with something general, like time management, asking for help will get you to a solution faster than you could ever hope to alone. If you want to avoid burnout, you’ll need to swallow your pride on occasion and reach out for help.
7. Make others laugh
You’ll find it hard to be glum and entertain unhappy thoughts when the people around you are excited and happy to be near you.
8. Make an escape list
An “escape list” is a list of everything you’d need to do in order to escape a situation that’s driving you nuts. In a work context, your escape list might include things like turning in a final presentation or asking for a raise.
9. Embrace a morning ritual
Are you starting your day on the wrong foot by waking up late, rushing about, and skipping out the door at the last minute? Try slowing down your morning instead.
10. Stop making excuses
Once you’ve given up on blaming others you’ll start seeing more of the good in your life.
11. Be accountable
The trick is find somebody you can trust to give the down and dirty on what you’re trying to do and how you’re moving forward. For best results, have your accountability partner NOT be a relative or somebody you’re dating. They typically won’t have the capacity for objective review of your progress. People who love you will often make excuses for you, and you want to avoid excuses at all costs.

With all that being said, sometimes one of the best things we can do is . . . to do less. Take something off of your list of "to do's" that really isn't required for life to go on, and just rest. Sometimes "sleep therapy" is just what the doctor ordered!

In a busy, stress filled, sometimes painful world, where we focus on doing more and better, sometimes we have to pull back and just do less in order to recharge, regenerate and rest, . . . so that we can be at our best.

May is National Mental Health Month!

May is Mental Health Month and the MHA is promoting a campaign called: "Life Your Life Well". They have some wonderful online resources available for relieving stress and promoting mental health and wellness at: http://www.mentalhealthamerica.net/go/may

Here are some quick, simple and easy tips on how to improve your mood, feelings, mental and emotional health, and . . . your life!:

• Get at least 8 hours of sleep each night.
• Get 20 - 30 minutes of light to moderate exercise daily.
• Set some new goals, ie. to have, be and do.
• Develop a "Personal Philosophy" or "Mission Statement".
• Laugh as much as you can! (Joke books, movies, emails . . . & share the laughter with others.)
• Love, give it . . . and receive it. (Sometimes a real challenge!)
• Get a pet. (Proven to reduce stress and increase joy, within reason.)
• Take a day off every week and just REST.
• Visit a new part of town, or get away for the weekend.
• Do what you love to do the most at least a couple of times a week.
• Count your blessings! Giving thanks audibly and writing down a list of everything you can be thankful for, then looking at it and thinking about it frequently can transform your life!
• Find the good in everything, even if it's something very small.
• Take a fun class or start a positive book.
• Spend time with loved ones, friends or family.
• Eat some chocolate! (One of my personal favorites.)
• Smile as much as you can, even if you don't feel like it! (It releases "feel good" hormones in your brain!)
• Speak Words of Encouragement, Praise, and Thanksgiving to as many people as you can, especially friends and family members, as often as you can!

These are just a few simple suggestions that can make life more enjoyable, letting a little more sunshine in to brighten your day. . . . and in the process, improve your health . . . and possibly even save your life!
